Community Crisis Prevention Service (CCPS)

CCPS has emerged as our flagship umbrella service which compliments the ‘Healthy Leeds’ plan to improve health and wellbeing in Leeds.

Our focus:

Our main focus is:

  • Crisis Prevention
  • Self-Care
  • Cultural Sensitivity

Our goals:

An enhanced quality of life

  • Mental health crisis prevention
  • Enhanced individual mental well-being
  • A reduction in levels of social isolation

Positive health behaviours

  • Enhanced confidence levels, self-esteem and ability to self-care
  • More effective use of Third Sector and mainstream services
  • Better access to non-medical social activities and support

Efficient use of healthcare resources

  • A reduction in demand for GP services
  • A reduction in hospital outpatient attendance
  • Better patient experience

CCPS provides person centred support to individuals, carers and relatives helping them to develop a library of self-help resources. Once an individual is engaged with the service they have access to a personalised package of care, based on individual needs and evidence-based practice that can include self-help interventions, complementary therapies, skills-based recovery programmes and a range of group activities to build social capital, wellbeing and reduce social isolation.

As a team, we have a wealth of experience and bring a broad range of expertise, knowledge and understanding that enable us to successfully deliver the service, having first-hand experience of delivering similar work. We also work collaboratively with other service providers, where applicable, to achieve this.

CCPS assists in managing symptomatic relief and also helps to build self-confidence and self-care skills within the local community. We also build upon other models that promote self-care.

We are able to deliver workshops to help explain the health benefits of self-help tools and encourage the community to play a part in maintaining their own health and well-being where appropriate.

The service helps provide emotional and practical support thus helping to meet the needs of those struggling with physical and low-level mental health issues.

Support is available to those in the community who are marginalised from society thus helping them to benefit from the interventions we provide.

The CCPS helps to address issues such as:

  • Bereavement
  • Vulnerable groups
  • Low moods
  • Anxiety
  • Stress
  • Panic attacks
  • Anger management
  • Sleep deprivation

We currently deliver and arrange for interventions such as:

  • Bereavement ‘Care Cafe’ peer support groups
  • IAAU vulnerable women’s group
  • Black Men’s Mental Health Service
  • Active listening sessions
  • Culturally sensitive counselling
  • Complementary therapies
  • Information exchanges
  • Social events and activities

The Need:

Chapeltown and Harehills are areas of high deprivation within the city of Leeds resulting in health inequalities and associated high levels of various forms of mental ill health.

Based in the heart of Chapeltown Leeds, the CCPS is a good example of how local co-production can work to develop services designed by the community for the community and, as such, we have earnt the respect and trust of community members enabling us to engage with them in ways that other organisations and services may struggle with.

As a result of community consultation, we identified the following as some of the main issues and stressors adversely affecting mental wellness in the community:

  • Bereavement
  • Stress/Anxiety
  • Low moods/Depression
  • Sleep problems
  • Low energy levels
  • Cravings (i.e., from alcohol or substances/medication)
  • Pain
